Terms & Conditions
1. Scope of Services
Clear Path Healthcare provides non-medical home care, healthcare consulting, and durable medical equipment (DME) coordination services. All services are delivered according to each client’s care plan and level of need, as determined during the intake and assessment process.
2. Service Agreement
Before services begin, clients (or their legal representative) must sign a Service Agreement outlining the type, frequency, and duration of care or consulting services. Clear Path Healthcare reserves the right to adjust services as client needs or circumstances change.
3. Payment Terms
Payment is due upon receipt of invoice unless otherwise stated in writing. Accepted payment methods include check, credit/debit card, or approved insurance coverage. Clients are responsible for any unpaid balances not covered by insurance or third-party payers.
4. Insurance and Authorizations
Clear Path Healthcare assists clients with insurance documentation and authorization requests; however, approval and payment decisions are made solely by the insurance carrier. Clients remain responsible for any non-covered services or denied claims.
5. Scheduling and Cancellations
To cancel or reschedule a service, clients must provide at least 24 hours’ notice. Missed appointments or cancellations without notice may result in a fee as stated in the Service Agreement.
6. Confidentiality
All client information is kept strictly confidential in compliance with HIPAA and state privacy regulations. Clear Path Healthcare will not share or release client information without written consent, except as required by law.
7. Equipment Use (DME)
Any DME provided or coordinated through Clear Path Healthcare must be used according to manufacturer instructions. Clients are responsible for proper care, maintenance, and safe usage of equipment.
8. Limitation of Liability
Clear Path Healthcare and its staff are committed to quality care. However, we are not responsible for injuries or damages resulting from factors beyond our control, including non-compliance with care plans, misuse of equipment, or unsafe home conditions.
9. Termination of Services
Either party may terminate services with written notice. Clear Path Healthcare reserves the right to discontinue services due to safety concerns, non-payment, or repeated scheduling issues.
